FORM R.S. IV

Pass of the transport of rectified spirit (including absolute alcohol) from the licensed premises of a person holding a sale licence or from a Government warehouse or distillery 1[or any licensed distillery or warehouse]1 to the licensed premises of a licensee holding a licence in Form R.S. II 1[R.S. VI or R.S. VIA]1

PART IV

2To be sent through the 3[Inspector or the Sub-Inspector of Prohibition and Excise, as the case may be], in whose jurisdiction the licensed premises of the person holding a sale licence or warehouse or distillery, as the case may be, is situated, to the 3[Inspector or the Sub-Inspector of Prohibition and Excise, as the case may be] of the place to which rectified spirit is to be transported]2

  1. Serial Number
  2. Name and address of the person transporting the rectified spirit (including absolute alcohol).
  3. Licence Number of the transporter.
  4. Quantity and description of the spirit.

  6. Name and address of the person holding a sale licence or Government warehouse or distillery
  7. Number and date of the requisition order.
  8. The purpose for which rectified spirit is to be transported.
  9. Date upto which the pass shall be valid.

This pass is granted under and subject to the provisions of the Bombay Prohibition Act, 1949, and the rules made there under authorising the above-mentioned transport, subject to the following condition, namely:-

The consignment shall not be broken in transit.

* These particulars are not to be given in the case of transport of rectified spirit by a person holding a licence in Form R.S. II, incidental to its purchase from person holding a sale licence in Form U.S. VI.
