What this form is for
FORM D. S. XXXII is a monthly register in which a licensee records the receipt, use and closing balance of denatured spirit used for bonafide medical, scientific and educational purposes. It is maintained by the holder of a Form D. S. IV licence, who must update it throughout the month to account for stock purchased, the quantity consumed, the purpose of use and the balance remaining. The requirement arises from condition 10 of Form D. S. IV, issued under the Bombay Denatured Spirit Rules, 1959.
