What this form is for
Form D. S. VIII is a written requisition for ordinary denatured spirit, addressed to a wholesaler or to the officer-in-charge of an Excise or Customs Bonded Warehouse. It is filed by a retailer or user who holds a licence for the sale or use of ordinary denatured spirit and needs to obtain a supply of it, stated in litres or bottles. The requisition is countersigned by a State Excise Officer. It is authorised by rules 38 and 43 of the Bombay Denatured Spirit Rules, 1959.
