Pass for Transport of Sacramental Wine

The pass runs in five parts across printed pages 1006–1010 (pdf 1038–1042). Same 6 particulars and conditions on every part; the difference is who retains each part:

• PART I — retained by the Officer-in-charge for his record
• PART II — handed over to the licensee, who shall retain it on his record
• PART III — handed over to the licensee, who shall forward it to the consignee; the consignee shall return it duly acknowledged to the issuing Officer
• PART IV — handed over to the licensee to accompany the consignment, and to be kept by the consignee for his record
• PART V — forwarded by the Officer-in-charge to the Inspector of Prohibition and Excise at the place of destination for verification

Particulars on every part

1. Serial No. ______
2. Name and Address of the licensee from whom the sacramental wine is to be transported.
3. Quantity of sacramental wine to be transported.
4. No. of authorisation held by the consignee (or No. of licence held by the consignee, for Part III).
5. Name and Address of the consignee.
6. Route.

This pass is granted under and subject to the provisions of the Bombay Prohibition Act, 1949, and the rules made thereunder authorising the transport subject to the following conditions, namely:—

1. The consignment shall not be broken in transit.
2. This pass will remain in force up to ______ 20 ______.

• PART I: "Compared with the Part II on ______ 20 ______" — Signature of the Officer issuing the pass.

• PART III: "Received the above quantity on ______ 20 ______" — Signature of the consignee (returned to issuing Officer).
• PART IV: "Received the above quantity on ______ 20 ______" — Signature of the consignee (retained by consignee).
• PART V: "Verified the consignment on ______ 20 ______" — Signature of the Excise Inspector at the place of destination.
