What this form is for
FORM F. L. X-D is a permit authorising an individual to purchase, possess, transport, use and consume mild liquor within the State of Maharashtra for a stated period. It is issued by the designated officer to a person who requires mild liquor for the preservation and maintenance of their health, and the holder signs a declaration to that effect when seeking a permit for a subsequent period. The form is prescribed under rule 70-F of the Bombay Foreign Liquor Rules, 1953, and the permit remains subject to the Bombay Prohibition Act, 1949, and the rules, regulations and orders made under it.

X-D (See rule 70-F) No. Shri/Smt./Kum. ......................................... age ................................ residing at ............................................................................ is hereby permitted to purchase, possess, transport, use and consume mild liquor within the State of Maharashtra, subject to the provisions of the Bombay Prohibition Act, 1949, and the rules, regulations and orders made hereunder, during the period commencing on the date on which this permit is granted and ending on .............................................
